ACA/1095-C Forms:
Any non-full-time and non-employee who are enrolled in the health care plan
may request a copy of the form 1095-C with the expectation that you will have a
physical copy within 30 days of the request.
Please submit requests for copies of form 1095-C to Dominicue Corvey, Human
Resources Manager. Please use one of the following methods to make the
